Affirmation Regulation

Prescribed form

1The prescribed form of solemn affirmation for the purposes of section 20 of the Evidence Act is as follows:

"I solemnly promise, affirm and declare that the evidence given by me to the court [or as the case may be] shall be the truth, the whole truth and nothing but the truth."

Alternate form

2Where the form prescribed in section 1 is not used, the prescribed form of solemn affirmation is the use of "I solemnly promise", "I solemnly affirm", "I solemnly declare" or words to like effect in place of "I swear", "I make oath" or words to like effect in an oath.
