1 Identification of Extended Health Care and Dental Plans:

Contract application

2 Persons to whom these contracts apply:

(a) employees whose eligibility has been established by a collective agreement, employer directive or order of the Lieutenant Governor in Council;

(b) eligible employees of British Columbia Ferry Services Inc. and Deas Pacific Marine Corporation;

(c) eligible employees of the British Columbia Buildings Corporation;

(d) eligible employees of the Emergency Health Services Commission excluded from bargaining units;

(e) members and eligible employees of the Legislative Assembly;

(f) eligible employees of the Labour Relations Board;

(g) eligible employees of the Science Council;

(h) eligible members of the Justice Development Commission, officers and employees employed under the Justice Administration Act;

(i) judges and judicial justices of the peace appointed under the Provincial Court Act;

(j) masters appointed under the Supreme Court Act;

(k) persons appointed under an Act as an officer of the Legislature;

(i) who work in a public service that is privatized by the Province disposing of it to the private sector, and who buy and subsequently work in the privatized venture, or

(t) the Union of Psychiatric Nurses of British Columbia as employer and its eligible employees;

Rates

3 (1) The following rates of contribution and coverage apply to those employees or persons described in section 2:

Extended Health Care Plan Maintains cost Nil
The plan is subject to a lifetime maximum payment of $50 000 per eligible employee and per eligible dependent.
Dental Plan    
100% of Part A
65% of Part B
55% of Part C
Maintains cost Nil
Part C is subject to a lifetime maximum payment of $3 500 per eligible employee and per eligible dependent.

(2) The following rates of contribution and coverage apply to those employees or persons described in section 2 (b):

The plan is subject to a lifetime maximum payment of $40 000 per eligible employee and per eligible dependent.

Part C is subject to a lifetime maximum payment of $5 000 per eligible employee and per eligible dependent.

Conditions

4 Conditions affecting those employees covered by these plans:

(a) employees granted leave of absence without pay for a period of a full calendar month or more may continue coverage for a maximum duration of 18 months, or in the case of educational leave to a maximum duration of 24 months, provided that the employee pays the full cost during the period of absence;

(b) when a regular employee becomes an auxiliary employee, benefits under these plans shall cease at the end of the month in which the change is made, except where continuation of benefits is provided for in an applicable collective agreement, employer directive, or order of the Lieutenant Governor in Council;

(c) when a covered employee transfers without a break in service from one group under these plans to another group also under these plans, coverage shall continue uninterrupted;

(d) when a covered employee has a break in service of less than one full calendar month and returns to service to either the same group or another group covered under these plans, coverage shall continue uninterrupted;

(e) the effective date of coverage under these plans for an employee who has already completed more than the 6 month qualifying period, but has not completed a registration card, is the first day of the month following the month in which the registration card is received by the carrier;

(f) for persons referred to in section 2 (s), coverage is limited to 6 months following the date the privatized venture begins operating in the private sector;

(g) regular or auxiliary employees who are on temporary layoff for a period of a full calendar month or more may continue coverage for the period specified in the applicable collective agreement, employer directive, or order of the Lieutenant Governor in council, provided that the employee pays the full cost during the period of layoff.
